If you find that you no longer need your Q doctor video appointment or would like to change the time/date, please can you let us know in plenty of time so the appointment can be used by someone else.

Your name
Your practice's name
Date and time of appointment you'd like to cancel

If you are looking to change your appointment you will need to book the new appointment yourself and we will cancel the one you no longer need.

Please note that we are only able to cancel video appointment booked through Q doctor  - you will need to contact your surgery to cancel or change any face-to-face or telephone consultations.
